Are you sure that you are using EPC2000 compliant forms?
The form identifiers are
EP2000 for EP1001E2K and
EP122K for EP1200E2K
- old non-compliant EPC 2000 forms are no longer supported.

With v4, this form will not appear anymore for new cases (only already sent cases can still be viewed). If you want to remove it already now - you can delete the respective line in your fmi.ini (and restart the application or server) - but you can then not view any existing cases of that form anymor...
